Practice Improvement Specialist – Home & Community Care Make a real impact by inspiring people, enlivening communities, and confronting injustice. Uniting is seeking an experienced Practice Improvement Specialist to lead person‑centred care and continuous improvement across our Home and Community Care services. This is a key leadership role where you’ll partner with operational and service delivery teams to embed contemporary practice, improve quality outcomes, and support sustainable change across Seniors Services. About The Role You will operate as a subject matter expert in practice development, supporting the adoption of person‑centred care principles and contemporary models of service delivery. Working closely with leaders and frontline teams, you will guide policy and practice improvements, develop frameworks and training, and use data and research to drive continuous improvement. Key Responsibilities Lead the adoption of person‑centred care across Home and Community Care Provide subject matter expertise in practice development and change management Guide policy, practice, and system improvement initiatives Collaborate with service delivery teams to implement new models of care Develop practice frameworks, standards, and training resources Use data, research, and sector trends to inform improvements Support quality, governance, and safe service delivery outcomes About You Bachelor’s degree in health or business (or equivalent experience) Experience in practice development, coaching, and change leadership Strong understanding of person‑centred and evidence‑based practice Excellent communication, problem‑solving, and organisational skills Commitment to inclusion, diversity, and high‑quality care Health professional registration, post‑graduate qualifications, or experience in service redesign, coaching, or process improvement will be highly regarded.
