Aboriginal Health Practitioner – Aged Care Duration: Full time, fixed term until 30 June 2027 Base Salary: $89,565 to $93,183 per annum (AHP 3.1 to 3.3) Darwin's only Aboriginal community-controlled health service 6 weeks annual leave + 5 paid days during Christmas shutdown Ongoing opportunity with career growth pathways This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT). Work within a multidisciplinary team to address the unique needs of the elders in our care. Who are we? Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the YilliRreung (Greater Darwin) Region in the Northern Territory. About the role As the Aboriginal Health Practitioner Aged Care, you will be working within a multidisciplinary team to address the unique needs of the elders in our care and bridge the gap between our aged care and clinic services ensuring continuity of care for each elder. Responsibilities Provide a range of clinical services including health assessments and care planning, all delivered with cultural sensitivity and in coordination with the Clinical Nurse Consultant Aged Care and DDHS multidisciplinary team. Develop, implement, and review individualised care plans in collaboration with the multidisciplinary team, ensuring care, services, and goals remain culturally centred and aligned with changing needs and preferences of each elder. Provide regular mentoring, guidance, and performance feedback to support worker staff, ensuring culturally safe, trauma‑aware, and healing‑informed practices that meet the needs of Aboriginal and Torres Strait Islander elders. Qualifications Minimum Cert IV Aboriginal and Torres Strait Islander Health Care (Practice) Current registration with AHPRA Minimum 3 years’ experience working in a Primary Health Care setting. Well‑developed knowledge of, and contemporary experience in, working with clients with chronic health conditions (e.g., diabetes, cancer, respiratory and/or cardiac disease) and relevant management strategies. Demonstrated competence working within multidisciplinary teams or independently, with strong organisational skills and the ability to plan, manage, and prioritise tasks. Ability to mentor and support junior staff. Knowledge of Aged Care Quality Standards and regulatory requirements in Aged Care. What we offer 6 weeks annual leave per annum 5 days paid leave during end of year shutdown (on top of annual leave). Salary packaging of up to $15,990 per annum.
