Occupational Therapist (Strengths Based Role) PCCS has an exciting opportunity for an Occupational Therapist who wants to make meaningful and positive impact by working with our clients and offering best practice OT service and a personalised care approach to people living with long term health conditions, including mental illness and other needs. Our mission is to optimise Health and Wellbeing for local communities and support young people and adults with simple to complex support needs including physical, social and mental health issues covering illness and injury. About the Opportunity Providing a high standard of clinical services to a range of age groups that includes client-focused assessment and therapy, equipment prescription, and home modifications. Develop care plans and monitor adherence to care plan, evaluate effectiveness, monitor client’s progress in a timely manner and facilitate any changes required. Ensure the efficient and effective use of resources, including setting priorities so those clients with the highest needs are given priority in the provision of services. Work closely with the multi-disciplinary team and senior management to support improved care planning and continuity of care across service boundaries. Coordinate the promotion of Primary Care support services and resources to relevant stakeholders. Selection Criteria Professional Registration AHPRA Experience in similar role is desirable Knowledge of disability & mental health services.
