Care Partner role at HammondCare About The Role The Care Partner works within the Mission and Mission in Action of HammondCare, supporting older adults to live independently and well in their own homes. This permanent, full‑time position (Monday to Friday) is based with the HammondCare at Home Team in Tuggerah. The role involves client‑centred care planning, clinical care management, and coordination with the Client Care Operations Manager to achieve strategic objectives and grow the client base. Responsibilities Work closely with clients, families, and multidisciplinary teams to develop and implement personalised care plans. Assist in providing services tailored to individual client needs, delivered within budget and service expectations, including ongoing assessment and care plan development in consultation with stakeholders. Be responsible for the care management of existing clients and client‑related KPIs. Ensure care plans are up to date and clients receive an appropriate type and amount of care corresponding to their needs, approvals, and funding. Provide professional day‑to‑day delivery of services in line with service expectations, ensuring staff understand individual client needs and preferences. Qualifications Certificate IV in Aged Care, Certificate IV in Disability, Certificate IV in Community Services, Diploma of Community Services (Case Management), or Diploma of Ageing Studies and Services. At least two years of experience in aged care, home care, or community care with a passion for client‑focused service delivery. Strong ability to develop, implement, and review personalised care plans. Excellent communication and relationship‑building skills, with a commitment to advocacy and person‑centred care. Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards, and the Commonwealth Home Support Programme. Strong problem‑solving, organisational, and coordination skills. Experience operating effectively in a multi‑site environment. Current driver’s licence with a safe driving record; willingness to travel between HammondCare sites and services.
