Care Partner
Vor 4 Tagen
Are you an experienced Case Manager or Care Coordinator ready to step into a more person‑centred, future‑focused role? At Odyssey Lifestyle Care Communities, we call it a Care Partner — because this role is about walking alongside residents and their supporters, not just “managing packages”. It’s about partnering with older people to achieve their goals, maintain independence, and live well in a community that feels like home. As our Care Partner, you’ll coordinate Support at Home and Private services, guide residents and families through their care journey, and champion rights, choice and dignity under the new Aged Care Act. You’ll be part of a pioneering team delivering goal‑directed, home‑based care within our Odyssey Robina Community that enhances lives while staying true to our mission: Ladies and Gentlemen serving Ladies and Gentlemen . Odyssey is a growing, owner‑operated organisation with exciting opportunities ahead. As we expand our communities and services, we’re committed to supporting our staff to grow with us. You’ll have regular access to the owner and executive team, with strong, hands‑on support and open communication. This isn’t a distant corporate structure — it’s a connected, values‑led environment where your input matters and your career can thrive. Whether you're looking to broaden your scope or build on specialised care‑partnering expertise, we offer clear pathways for progression, backed by mentorship and genuine development opportunities. What You’ll Do Partner with residents and their chosen supporters to understand what matters to them and co‑design wellness, reablement and lifestyle goals under the Support at Home program. Develop, implement and review individual care and support plans that reflect goals, preferences, risks and changing needs, using tools such as care plans, assessments and budget tools. Coordinate and monitor safe, timely, effective and cost‑efficient services that uphold residents’ independence, dignity and choice. Build resident service models, and prepare care service agreements, ensuring fees and inclusions are transparent and fully discussed with residents and/or families. Act as the key point of contact for changes, cancellations and ad‑hoc service requests, responding promptly and communicating clearly with residents, families and the care team. Regularly review Support at Home budgets to ensure adequate services are in place to meet residents’ goals while avoiding overspend of allocated funds. Advocate for residents’ rights and preferences in line with the new Aged Care Act and the Code of Conduct for Aged Care, supporting informed decision‑making. Build strong, collaborative relationships with the Wellness Manager, clinical team, Community Manager, external providers and the Finance Team to ensure seamless service delivery and accurate billing. Contribute to quality improvement projects, audits and reporting requirements in line with Odyssey’s Quality Management System and legislative frameworks. Assist with new resident assessments, onboarding and move‑in documentation as required, and take on additional duties as services grow and evolve. What You Bring Minimum 2 years’ experience in a case management, care coordination or similar role within aged care, community care or health services. Strong understanding of person‑centred, home‑based care and the emerging Support at Home framework, with working knowledge of My Aged Care pathways and systems. Good understanding of the Aged Care Quality Standards and the regulatory environment, with an interest in the new Aged Care Act and strengthened standards. Demonstrated ability to build trust with residents and families, manage complex situations, and find practical, resident‑focused solutions. Experience planning and managing individual budgets, monitoring spend, and balancing value for money with quality outcomes. Confident using digital systems and technology (MS Office, care management systems, smartphones, tablets and laptops). Clear, confident written and verbal communication skills, with strong documentation capability and the ability to accurately record and track care management minutes in an organised, systematic way. Current First Aid/CPR (or willingness to obtain), influenza vaccination, valid working rights and a satisfactory National Police Check (or willingness to obtain). Bachelor of Nursing Experience in retirement living, home care packages/support at home, or flexible in‑home support services. Knowledge of the Retirement Villages Act QLD Why Odyssey? We’re not your typical Care Community — we’re an innovating Lifestyle Resort with warmth, independence, and clinical excellence at the core. Enjoy exceptional working conditions, with thoughtfully designed facilities, natural light, high‑end amenities, and purpose‑built spaces that support quality care delivery. Be part of a values‑driven organisation where your contributions are recognised, your voice is heard, and your wellbeing matters. Join a workplace where innovation, respect and excellence are more than words – they’re lived every day. Make a lasting impact in a values‑led community that truly puts people first. #J-18808-Ljbffr
