About The Facility Major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and several residential aged care facilities across the region. Comprehensive mental health, community health and dental services are delivered across the Barwon South West Region. About The Role As an assessment clinician you will work independently within a multidisciplinary team to deliver the Aged Care Assessment Program in the VIC 3 region. Your primary responsibilities include conducting comprehensive assessments across physical, medical, psychological, cultural, and social care dimensions, offering consumers appropriate service options, collaborating with clients to assess needs, goals and preferences, focusing on wellness and reablement to promote independence, and providing information and referrals to suitable services (e.g., residential care, respite, Support at Home, DVA, mental health, NDIS).
