About The Role As a Nurse Practitioner (NP) in our Palliative Care Service, you will bring high‑level clinical expertise to support complex and evolving care needs. You’ll work across residential aged care, hospital in the home (HiTH), and community settings, including ambulatory clinics. Responsibilities Provide expert clinical assessments and implement advanced care plans Lead initiatives in evidence‑based practice and clinical service planning Actively contribute to quality improvement and mentoring of staff Collaborate with the Community Palliative Care Team (CPCT) to enhance patient outcomes across care environments Qualifications and Experience Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse who is endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia. Demonstrated ability to work independently and in the absence of a Medical Practitioner to provide quality care, and to work collaboratively as part of a multidisciplinary team. High level knowledge and advanced clinical nursing skills, including comprehensive assessment, interpretation of diagnostics, implementation of treatment modalities and relevant referral skills applied to challenging, complex and changing situations. Understanding of the palliative care delivery system and ability to lead and implement changes and develop direction in clinical practice through evidence‑based research and service planning. Proven expertise in complex client‑centred consultancy, practice development activities including mentoring, education, and policy development. Advanced clinical leadership, negotiation, liaison, interpersonal and problem‑solving skills across the palliative care service that includes acute, sub‑acute and aged care sectors.
