SLHD End of Life Care Coordinator (Clinical Nurse Consultant Grade 2) Join to apply for the SLHD End of Life Care Coordinator (Clinical Nurse Consultant Grade 2) role at NSW Health Employment Type: Permanent Full Time; 38 hours per weekPosition Classification: Clinical Nurse Consultant Grade 2Remuneration: $135,826 - $138,534 per annum + Annual leave loading + SuperannuationLocation: Concord Repatriation General Hospital Requisition ID: REQ About the Role The SLHD End of Life Care Coordinator (EoLCC) is a dynamic position responsible for clinical leadership, safety, quality improvement and education to support the delivery of end of life care across the SLHD. The position will provide expert consultancy to facilitate evidence-based practices and advanced care planning, and delivery of systems to enhance end of life care which are high quality and person-centred. Responsibilities Provide expert clinical leadership and consultancy across end of life care. Lead safety, quality improvement and education initiatives. Facilitate evidence-based practices and advanced care planning. Deliver systems enhancements for high quality, person-centred end of life care. Qualifications Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA). Minimum five (5) years full-time equivalent post-registration experience. At least three (3) years experience in specialty field in Palliative Care, Cancer Care, Advanced Nursing, or Aged Care.
