We are establishing an aged care outreach service for older people with acute illness who are at risk of hospitalisation. The service will have a network of staff based at multiple locations across Western NSW and will provide a hybrid face to face and telehealth outreach service. An exciting opportunity exists to manage the development and implementation of this new model of care in a rural context. You will join and lead a team which includes Geriatricians, Nurse Practitioners, Registered Nurses, a Pharmacist and Administration staff and will work closely with the Aged Care Team, Clinical staff in our hospitals and community settings as well as staff in Residential Aged Care Homes. Your work will be a mix of outreach into residential aged care, outpatient clinics and acute care consultations. The service will initially operate between 08:00am -17:00pm, Monday to Friday with the option to extend hours as the service develops.
