You’ll lead a team of assessment professionals across the Wide Bay region who carry out person‑centred assessments that follow a clear framework and help shape individualised care planning. You’ll oversee day‑to‑day operations, guide your team through complex cases, and maintain a small caseload yourself to stay close to the work. You’ll make sure assessments are delivered on time, entered accurately, and meet the standards that help people get the right support at the right time. This full‑time role (Monday to Friday) includes some travel in the Wide Bay region to connect with your team, customers, and stakeholders and you’ll also enjoy catching up with your General Manager and fellow Team Leaders online as you work collaboratively to share insights, solve challenges, and make sure our assessment services stay consistent, responsive, and grounded in what matters most, helping people live well at home. You’ll thrive in this role if you Bring proven leadership experience in guiding a team with clarity, care, and consistency to deliver work that meets defined standards. Hold a tertiary qualification in a health‑related discipline, with current AHPRA registration or eligibility for a relevant professional association. Bring experience as an Aged Care Assessor (a bonus) and a sound understanding of the aged care landscape, including the Support at Home model, funding, guidelines, and legislation, so you can confidently support your team through complex cases. Value continuous improvement and know how to turn feedback into better ways of working. Build strong, respectful relationships with customers, colleagues, and community partners, because good care starts with good communication. Have a valid Driver’s Licence and reliable vehicle for work. Hold current First Aid, CPR, and a National Criminal History Check (or be willing to obtain them) and meet the necessary checks and declarations required for the role.
