About the Opportunity We are looking for a skilled and experienced Support Coordinator to join our team, providing services across the Illawarra and Greater Sydney regions. This role is offered full-time, though we are open to part-time candidates. Enjoy a hybrid mix of working from home, meeting participants in their homes and communities, and collaborating in person with your team. This hands‑on role supports NDIS participants to navigate services, make informed choices, and stay connected to quality supports. As a key link between participants, families, providers, and the NDIS, you'll help people understand and implement their plans, access services aligned with their goals, and build capacity for independence. This role suits someone who is a natural communicator and critical thinker, with a genuine passion for improving the lives of people with disability. Your Impact in Action Participant Support & Planning Work collaboratively with participants to understand their goals, needs, and preferences, and support them to navigate, coordinate, and implement their NDIS plans and budgets so funding is used effectively and aligned with their goals. Develop and regularly review tailored support plans in partnership with participants, families, and their trusted networks. Develop and review crisis response and risk management plans, ensuring participants feel informed, supported, and prepared. Facilitate plan reviews and reassessments with care, ensuring evidence genuinely reflects each participant’s progress, achievements, and next steps. Service Coordination and Implementation Connect participants with NDIS funded, mainstream, and community supports that promote independence, wellbeing, and genuine inclusion. Ensure all services align with participant goals, funding, and NDIS compliance standards – maintaining quality, safety, and accountability at every step. Capacity Building and Empowerment Build participants’ knowledge, skills, and confidence to navigate the NDIS and wider community systems with independence. Support and guide participants to coordinate and manage their own supports in ways that feel sustainable and empowering. Encourage self‑advocacy and informed decision‑making, providing practical tools and guidance for budgeting, planning, and expressing their needs. Share clear, accessible information about rights, options, and advocacy pathways to promote genuine choice and control. Quality, Compliance and Safeguarding Uphold the NDIS Code of Conduct, Practice Standards, and all legislative obligations, ensuring every service remains ethical, safe, and participant‑led. Maintain accurate, timely, and person‑centred documentation that meets privacy, audit, and reporting standards with care and integrity. Monitor participants’ wellbeing and circumstances, responding proactively to changes in risk or safety through appropriate escalation and safeguarding actions. Support compliance with all safeguarding, privacy, and mandatory reporting obligations. Engage in supervision, reflective practice, and ongoing professional development, continually building knowledge and skills to deliver best‑practice support. Contribute to continuous improvement, identifying service gaps and opportunities to strengthen quality, collaboration, and participant outcomes. Collaboration and Stakeholder Engagement Work collaboratively with allied health professionals, service providers and informal supports to deliver coordinated, person‑centred care. Facilitate open, transparent communication between all stakeholders to build trust, consistency, and shared understanding. Represent Care Focused with professionalism and integrity at interagency meetings, community forums, and sector events. Support and guide providers to align their services with each participant’s goals and funding, promoting quality, accountability, and genuine collaboration. Crisis Management & Problem Solving Provide responsive, participant‑focused support during times of crisis, ensuring safety, stability, and continuity of care. Identify and manage risks proactively, escalating to appropriate services when support needs exceed scope. Apply trauma‑informed, strengths‑based approaches to guide problem‑solving and engagement, helping participants regain confidence and control. What You Will Need NDIS Worker Screening Clearance (or willingness to obtain) Paid Working with Children Check (or willingness to obtain) NDIS Worker Orientation Module Certificate of Completion Current Driver Licence Reliable, roadworthy vehicle with comprehensive insurance Legal right to work in Australia Your Qualifications & Experience Tertiary qualification in Disability Services, Community Services, Mental Health, or a related discipline; or extensive relevant experience in the disability or community sector. Minimum of 2 years’ experience in Support Coordination, Case Management, or a similar role within the NDIS or community services sector. Strong understanding of the NDIS Price Guide and funding categories, with demonstrated ability to assist participants in managing and monitoring their plan budgets effectively. Proven experience supporting participants with diverse and complex needs, using a trauma‑informed and person‑centred approach. Excellent administrative and record‑keeping skills, with proficiency in Microsoft 365 and client management systems (knowledge of the MYP system is an advantage). Ready to Join Us?
