About Us The Connected Care team is part of the System Performance Support Branch and provides expert advice on issues that may impact health system performance. We identify opportunities for performance improvement and promote the implementation of state‑wide patient flow initiatives that improve and enhance the delivery of high‑quality, efficient and effective health services for the NSW community. Key priorities include hospital‑in‑the‑home reform and the implementation of the NSW Health Virtual Care Strategy. You will manage and oversee reform projects, developing project plans, evaluating outcomes, coordinating resources, monitoring progress, analysing outcome data, meeting reporting requirements and supporting project‑related activities to ensure outcomes are achieved on time, to quality standards and within scope. You will support the delivery of timely and evidence‑based strategic advice to Directors and other executive stakeholders on Virtual Care, HITH and ED Relief reform to support informed decision making and development of projects and strategies. What You’ll Be Doing Lead the team responsible for providing strategic direction to reform how NSW delivers clinical care outside of traditional hospital settings and embed the use of virtual care across health services. About You Providing strategic direction to enhance the care that can be delivered in acute out‑of‑hospital care models Experience leading statewide projects Experienced change manager with advanced skills in working with different stakeholders Experience leading a multidisciplinary team Ability to be agile to the changing needs of the system How To Apply Please review the role description and submit your up‑to‑date resume (up to 5 pages) and cover letter (1‑2 pages) and answer the two target questions below.
