About the role Part‑time (0.8) fixed term opportunity for 12 months with possibility to extend, based in Moss Vale, NSW. Reporting directly to the Team Leader Client Services, you will be responsible for: Facilitating individual and group education, information, and support to people living with dementia, their families, and carers Supporting clients in navigating aged care, disability, and health services by providing referrals and guidance Facilitating connections between people living with dementia and their carers through support groups and other engagement opportunities Ensuring service delivery is person‑centred, empowering clients to take an active role in their care Contributing to continuous improvement by identifying gaps in service delivery and sharing insights with leadership Building and maintaining partnerships with community organisations and service providers to enhance support options Maintaining accurate client records and data in compliance with professional and legal standards Occasional travel required to support regional groups already established in a variety of locations What is in it for you? About you To be successful in this role, we are looking for someone with the following: Formal qualification in a related field of either Social Work, Allied Health, Nursing and/or significant experience working with clients in the delivery of a community-based service or within a health-related field Demonstrated experience and understanding of the impacts of dementia for individuals, carers, and their support networks Demonstrated experience and knowledge of group facilitation and group dynamics. Experience in facilitating individual and group work with clients in a community setting is desirable Embraces new ideas and change initiatives Achieves results through a commitment to quality, planning and efficient resourcing Communicates clearly and openly with different audiences #J-18808-Ljbffr
