Home Dialysis Nurse Practitioner at Alfred Health Alfred Health is a leader in health care delivery, improvement, research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services. On 1 January 2026, Alfred Health will come together with Kooweerup Regional Health Service, Peninsula Health, Bass Coast Health and Gippsland Southern Health Service to create Bayside Health, a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians. The merger will strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health. Permanent Full time – 80hrs per fortnight About the Role The Home Dialysis Nurse Practitioner (NP) operates both autonomously and collaboratively within a multidisciplinary renal care team to deliver comprehensive, patient‑centred care focused on home dialysis therapies, including home haemodialysis (HHD) and peritoneal dialysis (PD). The NP supports patients throughout the entire care continuum, from modality selection and initiation to ongoing management, complication prevention and care transitions, with the goal of optimising clinical outcomes and enhancing quality of life. About You Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia via AHPRA. Minimum of 5 years experience in renal nursing. Post‑graduate Masters of Nurse Practitioner qualification. Motivated, autonomous with an ability to work collaboratively with departments across many health services and community.
