We provide a wide range of supports including life-stage transitions, personal activities assistance, transport, community nursing care, life-skills development, centre / group activities, household tasks, innovative community participation, psychosocial recovery coaching, support coordination and supported independent living. We are now looking for a dedicated Community Worker to join our Melbourne team. Role Overview : As a Community Worker you will be responsible for delivering high-quality support services to clients of Hiba Healthcare in line with client-goals, organisational values and regulatory requirements. Your work will involve direct engagement with participants, fostering independence, assisting with daily living, facilitating community participation and supporting meaningful connections and life skills. Key Responsibilities : Provide person-centred support to participants in line with their individual support plans, enabling them to achieve personal goals and maximise independence. Assist with daily living tasks, community access, travel / transport, household routines and life-skills development. Facilitate community participation, group / centre activities, psychosocial recovery coaching and other supports as needed. Build rapport with participants, families and other stakeholders, maintaining respectful, respectful and inclusive communication. Work collaboratively within a multidisciplinary team to ensure holistic care and support. Monitor and document progress, incidents and outcomes in line with organisational policies and compliance / regulatory standards. Uphold and promote the organisation’s values of ability-based approach, compassion and inclusion. Contribute to continuous improvement processes, providing feedback and suggestions for enhancing service delivery. Ensure the safety, dignity and rights of participants at all times. Key Selection Criteria : Diploma in Community Service (or equivalent) is essential. Minimum of 1 year of relevant experience in community services, disability support, or a related field. Strong understanding of person-centred support, life-skills development and community participation. Excellent interpersonal and communication skills, with the ability to engage effectively with participants, families and team members. Ability to work independently and as part of a team, with strong organisational and time-management skills. Sound documentation and reporting skills, with commitment to high standards of compliance, confidentiality and professional conduct. Flexible and adaptable approach, willing to respond to a dynamic service environment. Current national police check (or willingness to obtain), and valid driver’s licence (as required).
