What you’ll be doing in this role? The responsibilities of a Care Partner are varied and each day brings new challenges. As a member of a larger team, you will oversee a portfolio of clients, ensuring that you provide the necessary support for them to safely stay at home while working towards their goals. To Be Successful, You Will Require Assessment skills: Conduct thorough assessments of individuals’ needs, including their physical, emotional, and social well‑being. Gather information about the person’s condition, medical history, support network, and any specific requirements to determine the appropriate level and type of care. Care Planning: Develop comprehensive care plans tailored to each individual’s unique needs and preferences based on the assessment. Consumer‑Focused: Place the consumer at the center of your work, prioritising their choices, preferences, and goals. Actively involve the individual in decision‑making and empower them to make informed choices about their care. Liaison with Health Professionals: Communicate and collaborate with doctors, nurses, therapists, and social workers to ensure the care plan is implemented effectively and any necessary adjustments or referrals are made. Visiting Consumer: Visit consumers in their homes to assess well‑being, monitor progress, and provide ongoing support. Build rapport, address concerns, and ensure quality of care. Empathy and Communication: Listen actively, understand needs and emotions, and communicate effectively to build trust and a positive relationship. Problem‑Solving and Advocacy: Identify challenges and find appropriate solutions. Advocate for consumer rights and navigate obstacles or barriers that may arise. Organisation and Time Management: Handle multiple cases simultaneously by excelling in organisational skills, prioritising tasks, and meeting deadlines. Compassion and Patience: Provide reassurance, encouragement, and emotional support to help individuals and families navigate difficult situations.
