Do you have a deep understanding of your local community and a knack for helping people navigate support services? If so, we are looking for a Carer Hub Navigator – who will be the welcoming face and guiding hand for carers in our new Carer Hubs. This role integrates community development and community engagement to raise awareness and accessibility to Carer Gateway services through our Carer Gateway Hub. The Carer Services Navigator is based in the Nambour region, providing a consistent presence and reliable support to carers with a no wrong door approach. You will work in collaboration with a team to build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to embed carer support locally. Responsibilities Oversee the Nambour Carer Hub, facilitating social interaction and promoting wellbeing, connection and belonging. Create seamless connections between Carers and vital community resources, cultivating meaningful relationships that support wellbeing and independence. Support Carers to thrive with a holistic approach, going beyond basic needs to help them achieve their full potential. Maintain a Carer-focused service with a no wrong door approach.
