Job Overview Matrix Providers is hiring a Clinical Pharmacist to join our team of talented professionals who provide health care services to Military Service Members and their families at Travis Air Force Base, California. Employment Details Employment Status: Full‑time coverage position (minimum of 12 weeks) Compensation: Hourly position paid bi‑weekly Schedule: Services provided Monday — Friday, 9‑hour shifts with an uncompensated 60‑minute lunch break Benefits Competitive financial package with a comprehensive insurance package including health, dental, vision, and life coverage Accrued Paid Time Off (PTO) Paid Holidays (outlined in handbook) 401(k) plan Qualifications Doctor of Pharmacy (Pharm.D.) from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE) Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists (ASHP) or American College of Clinical Pharmacy (ACCP), or documentation of appropriate education, training, and/or continuing medical education (CME) in the practice of clinical pharmacy Current, full, active, and unrestricted license to practice as a pharmacist Basic Life Support for Healthcare Providers certification (American Heart Association or American Red Cross Healthcare Provider Course) Experience & Knowledge Requirements Minimum of three years of experience within the last five years working as an infectious disease clinical pharmacist, or completion of a post‑graduate year two specialty pharmacy practice residency in Infectious Diseases Expert knowledge of evaluative, analytical, investigative, qualitative and quantitative techniques to conduct review and analysis of pharmacoeconomic issues such as cost‑effectiveness, cost minimization, quality‑adjusted life‑years, overall drug therapy costs, budget forecasting, and review of automated ordering for performance pricing and policy recommendation Experience in planning, organizing, and directing outcomes of research studies and economic analyses within the pharmacy and advising management on budget and program implications Knowledge of complex pharmacological protocols to include use, method of administration, dosing, schedule, adverse reactions, and complications for acute episodic and chronic illnesses Mastery to apply experimental theories and new developments to the resolution of complex health‑care problems not susceptible to treatment by accepted methods, or to make decisions or recommendations that significantly change or develop protocols Knowledge of the full range of professional medical principles and practices and their application to provide a broad range of direct primary health‑care services (e.g., recognize signs and symptoms of illness, interpret lab results, and assess patients’ medical conditions and health‑maintenance status) Knowledge and ability to apply emergency and resuscitative measures; independently make decisions and implement proper emergency measures after identifying and interpreting life‑threatening conditions; initiate life‑saving measures including basic cardiopulmonary resuscitation, calculating dosages and mixing potent and potentially lethal drugs, and administering these drugs Thorough knowledge to provide preventive anticipatory care to adults in nutrition, care of common illnesses, growth and development Knowledge of principles of history taking, physical and psychosocial assessment, using the problem‑oriented approach Knowledge of a variety of common, acute and chronic illnesses, their causes, prevention, treatment and other conditions which may affect the course of the illness Knowledge of medical care and treatment sufficient to perform complete physical examinations and administer a variety of medical treatments Knowledge of and the ability to apply principles of infection control and safety Ability to communicate effectively in writing, including patient records, standing operating procedures, staff records, reports, minutes, electronic means, and databases Utilizes various automated systems including Composite Healthcare System (CHCS), Ambulatory Data Module (ADM), Outlook, Computerized Information System (CIS), and Preventive Healthcare Application (PHCA) High level of interpersonal skills to work closely with patients and the Military Treatment Facility (MTF) staff; possess skill in the utilization of good human‑relationship principles and techniques to resolve issues and ensure successful implementation of proposals Skilled in briefings and preparation of study reports and briefing charts; capable of handling proposals that involve significant agency resources, extensive procedural changes or potential conflicts with other activities Core Job Duties Perform duties as a Clinical Pharmacist in direct support of the provision of healthcare to patients admitted to the inpatient wards and intensive care units of the hospital in addition to outpatient clinics Co‑lead the Antimicrobial Stewardship Program (ASP) with the ASP Medical Director as part of a multidisciplinary team Monitor and manage the development, implementation, and monitoring of the ASP, including antibiotic guidelines, clinical pathways, policy development, formulary management, restriction criteria, educational efforts, antimicrobial usage, and resistance monitoring Advise physicians, medical residents, nurses, and patients on antimicrobial use to optimize patient outcomes and ensure safe, cost‑effective use Develop core strategies for the ASP, including key performance indicators related to antimicrobial utilization and stewardship Review and develop best‑practice recommendations for all areas of antimicrobial management Maintain effective communication with leadership teams on ASP and assist in the preparation and implementation of pharmacy staff competencies related to antimicrobial management Plan and provide appropriate treatments, initiating and modifying selective therapies Write prescriptions for legend and over‑the‑counter medications in accordance with pharmacy list approved by the Pharmacy and Therapeutic Committee Perform treatments authorized by protocol or physician consultant Periodic evaluation of the proper utilization and dispensing of medications, coordinated by a physician preceptor Refer patients to supervisory physicians, other hospital departments, support services, or community social welfare agencies as appropriate Serve as the expert subject‑matter consultant (SMC) for drug therapy and pharmacy service issues, and conduct special quality‑assurance studies Perform pharmacoeconomic analyses Provide advice, professional consultation, opinions, and written technical studies and reviews in support of the various boards and committees within DGMC involved with evaluation of patient care and treatment Attend Department or Hospital board or committee meetings Lead and participate in the ward and pharmacy utilization‑management and quality‑assurance processes Develop and ensure compliance with defined protocols Attend continuing medical education (CME) courses and maintain specialty‑specific CME sufficient to satisfy licensing agency and Joint Commission requirements Conduct ward orientation for new medical, nursing, allied health, and pharmacy students, interns, and residents in diagnosis and treatment of patients Teach in formal lecture settings, mentor and conduct final evaluation feedback Prepare, conduct, or oversee training of physicians, pharmacists, and other health‑care providers on the use of antimicrobials Monitor training of residents and interns #J-18808-Ljbffr
