Salary: ENC/END- $63,992.00 - $74,831.00 p.a. + Superannuation & Salary Sacrifice Benefits Salary: ASO3 - $67,589.00 - $72,033.00 p.a. + Superannuation & Salary Sacrifice Benefits Salary: OPS3- $67,589.00 - $72,033.00 p.a. + Superannuation & Salary Sacrifice Benefits Full Time, Contracted Role up to30 Jun 2026 with the possibility of extension About the Role As a Home Support Assessor within the Single Assessment Service, you will play a vital role in supporting older South Australians to access the care and services they need to live safely and independently at home. This multiclass position (EN/END, ASO3, OPS3) offers opportunities across three classifications, with appointment based on your qualifications, skills and experience. In this rewarding role, you will conduct home-based face-to-face assessments for clients referred via My Aged Care, using the Integrated Assessment Tool (IAT) and the Aged Care Assessor application. Working closely with clients and their carers, you will identify needs, establish meaningful goals, and develop tailored support plans that connect individuals to the most appropriate Commonwealth and community services. Your work will be central to ensuring clients receive accurate, timely and person-centred guidance. You will enter and maintain high-quality data within the My Aged Care Assessor portal, provide clear information on aged care pathways, and support informed decision-making for clients and families. Collaboration is at the heart of this role, and you will liaise with carers, healthcare professionals, service providers and stakeholders to ensure seamless coordination of care. Operating within established guidelines, you will meet assessor benchmarks and KPIs set by the Commonwealth Government and SA Health, contributing to a service that has a lasting and positive impact on the wellbeing of older people in our community. Experience Experience within the aged and/or community care sector, including using the My Aged Care system, is essential. You will have a background working with people with diverse and complex needs—such as the frail aged, individuals with dementia or disability, or those who are terminally ill—and experience collaborating within team environments and supporting clients and carers. Knowledge Understanding of the aged and community care sector, including service pathways, provider options, and relevant legislation, policies and procedures. Knowledge of WHS responsibilities, the Charter of Health and Community Services Rights, National Safety and Quality Health Service Standards, Microsoft applications, Consumer Directed Care and broader ageing-related issues will further support your success in the role.
