Job Description The Home Care Package (HCP) team provide individually planned and coordinated suite of support and services designed to assist older people who need support to remain living independently, in their own homes. Casual position covering the Greater Kempsey region (Kempsey, Gladstone, Yarravel, Kundabung, South West Rocks, Smithtown). Paying up to $36.46ph + 25% casual loading + Not-for-Profit Salary Packaging + Time and travel paid between clients + In shift travel reimbursements. Your role your impact We’re on the lookout for a compassionate and dedicated Support Worker (new or experienced) qualified with a minimal Cert III in Aged Care, to join our team and make a real difference in the lives of older adults. As part of our Home Care Packages program, you’ll have the opportunity to provide tailored, high-quality support that empowers clients to live their best, most independent lives. Empowering Independence: Delivering personalized support to help older adult clients maintain and grow their independence, ensuring they lead fulfilling and enriched lives. Making a Positive Impact: Implementing individual service plans, healthcare plans, and positive behavior support strategies to promote well-being and quality of life. Creating Connection: Offering companionship, engaging conversation, and emotional support, while assisting with personal care needs in the comfort of the client's home. Helping with Travel: Assisting clients with transportation to important appointments, ensuring they stay connected and supported in their healthcare journey. Preferred experience working as a support worker (on job training provided). The ability to follow and implement individual service plans, healthcare, and positive behaviour support plans. A desire to use your skills and experience creating fun and interesting activities that support older adults to engage more consistently with their community and peers. A valid driver’s licence.
