MACARF Heart Failure and Supportive Care Nurse Practitioner Join to apply for the MACARF Heart Failure and Supportive Care Nurse Practitioner role at NSW Health Experienced Cardiac Nurses – an exciting opportunity to provide clinical expertise as a Nurse Practitioner (NP) in Heart Failure Supportive Care. This role will be part of a multidisciplinary team approach to deliver supportive care to patients with advanced heart failure. Where you'll be working : The NP Heart Failure Supportive Care will work within the NSLHD – MACARF Heart Failure service with support from the NSW Palliative Care Department and NSLHD Palliative Care Network, in collaboration with other Acute Services, Hammond Care Palliative Care services and Northern Sydney Home Nursing Service. What You'll Be Doing : The Nurse Practitioner will provide expert palliative care assessments and management associated with usual heart failure care. The NP is an expert in heart failure management with palliative care skills and will support the educational and professional development of nursing groups, allied health staff, patients, carers and their families. Key functions include acting as a conduit for heart failure patients transitioning to end‑of‑life care, establishing links with community palliative and aged care services, and independently managing a range of patient palliative/end‑of‑life care options within a defined scope of practice. The NP will conduct comprehensive systematic patient assessments and determine appropriate interventions, referring to specialised consultants when beyond scope. Candidates Will Need To Meet The Following Criteria : Current registration with AHPRA and authorised to practice as a Nurse Practitioner by the NSW Nurses & Midwifery Registration Board (AHPRA) or working towards such a qualification. If transitional, 5 years full‑time equivalent post‑registration experience, with at least 3 years full‑time equivalent clinical experience in the specialty of the Nurse Practitioner role. Demonstrated clinical competency in advanced practice management in Cardiac Failure with an interest in Supportive and Palliative Care. Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines. Demonstrated ability to work autonomously and have effective communication and negotiation skills. Current Driver's Licence.
