Responsibilities Provide person‑centred care and support to clients with disability in home and community settings Assist clients with daily living activities including personal care, meal preparation and mobility Promote social inclusion and participation through community access and engagement programmes Support clients to develop independent living skills and achieve their individual NDIS goals Administer medication safely and monitor clients’ health and wellbeing as required Maintain accurate records and progress notes for each client in accordance with NDIS requirements Assist clients to access disability support services, medical appointments and community resources Provide emotional support and companionship to enhance quality of life and wellbeing Work collaboratively with families, therapists and multidisciplinary teams to ensure cohesive care Follow all safety, health and environmental procedures to ensure a safe working environment Report incidents, risks or behavioural changes promptly in line with company policies Participate in ongoing training, meetings and professional development activities as directed Support clients with transport and participation in recreational and social activities Promote the values of empathy, respect and accountability in every aspect of client support Ensure confidentiality and uphold the dignity and rights of all clients at all times Qualifications and Experience Certificate III or IV in Individual Support, Disability or Community Services, or equivalent qualification Proven experience providing support to people with disability in home or community settings Strong understanding of person‑centred care and the principles of the NDIS framework Excellent communication and interpersonal skills to build trust with clients and families Ability to work autonomously and as part of a team across rotating shifts including weekends Commitment to professional boundaries, ethical conduct and maintaining client confidentiality Valid driver’s licence and reliable vehicle for client transport where required Required clearances: First Aid, CPR and Working With Children Check or NDIS Worker Screening clearance.
