About the Role The newly integrated Adult and Older Adult Service combines AMAH Adult Community Mental Health and Caulfield Aged and Liaison Psychiatry Services to provide comprehensive, person‑centred support for adults and older adults. We offer a wide range of therapeutic, addiction, trauma‑informed, and family violence services. In line with the Royal Commission into Victoria’s Mental Health recommendations and the Victorian Mental Health and Wellbeing Act 2022, we are expanding our services to provide specialised expertise and broad‑spectrum care, including pathways for substance use, rehabilitation, recovery, and trauma‑focused care. Our approach focuses on integrated treatment, education, peer support, care planning, and navigation, supporting individuals at every stage of their recovery journey. You’ll have the opportunity to work in areas such as: Navigations and Continuing Care: Brief Intervention, Access and Intake, CMHT, Physical Health, and Family Services Adult Rehabilitation and Recovery: MSTS, CCU, PARC, HOPS, and SECU Aged/Older Adult Mental Health: MAPS, Caulfield Hospital Community Liaison, and inpatient units Wellbeing and Therapeutic Services: Psychological Trauma and Capacity Building initiatives What You’ll Be Doing You will join a team of four other Senior Clinicians and a Senior Mental Health Support Worker who work together to provide the AMAH Adult service with a single point of entry for consumers between the age of 25 – 65. You will assess referrals and will assist with short‑term support, consultations and liaison with the community. Your team is responsible for providing face‑to‑face one‑off assessments as well as secondary consultation and education to primary health providers. The team assists with internal referrals and the movement of consumers throughout the community program.
