We are currently seeking a dedicated Practice Support Worker to join our Foster & Kinship team located in our Toowoomba region on a permanent part time basis (4 x days per week.) A Practice Support Worker works collaboratively with the case management team to support and maximise the effectiveness of support plans, to ensure planned and permanent goals for children or young people (CYP) are achieved. The role involves working with CYP with a range of developmental, behavioural and psychological needs and their families, carers and support teams, providing support through coaching, mentoring, training and modelling; along with the facilitation and coordination of available supports. Responsibilities Actively support the implementation of the plans by providing direct support to the child or young person and their families and/or carers Undertake direct support functions as required and under authority to facilitate plan goals including supporting contact, transportation, behavioural observation and recording, in-home and in-community support to CYP, carers and permanent plan stakeholders Actively support the implementation of the plans by facilitating and coordinating supports available in the community Support the implementation of sympathetic and effective client feedback and monitoring systems, providing support for their effective use and review Provide training, coaching, mentoring and other support for the implementation of strategies and plans to bring about behavioural change in children, parents and carers Contribute to the monitoring and review the assessments and interventions for effectiveness and validity Event Planning and Coordination Support the Foster and Equipping Teams with assisting in suitable tasks to ensure adequate service delivery Complete and regularly update session plans, outlining support provided support for CYP and households Complete case notes whilst outlining observations and implemented supports About you To be successful in this role it is preferred that you candemonstrate knowledge and experience relevant to working alongside young peopleresidential and/or youth programs, together with: A relevant qualification and/or Diploma in related fields of Human Services, community development is desirable, but not essential Demonstrated experience in developing and building positive rapport with a range of stakeholders, including community members, young people, carers, fellow staff Experience in working with indigenous communities and culturally diverse people and communities Understanding or direct experience in child protection or similar industry Knowledge of complex trauma, neglect and other factors related to why children and young people are placed in out of home care Compulsory screening checks National Police Check Blue Card LCS Suitability Check What we can offer you Ongoing training and career development opportunities including Hope and Healing Certificate Flexible working and family friendly environment Professional supervision and support Employee Assistance Program Our Commitment to Cultural Safety We are committed to creating a culturally safe and inclusive environment for all children, young people, families, and staff.
