Support Coordinator/Psychosocial Recovery Coach

Support Coordinator/Psychosocial Recovery Coach Stellar Support Coordination is seeking a passionate and experienced Support Coordinator to join our Wodonga-based team on a permanent part-time basis, with the potential to expand to a full-time role in future. About the Role You will join our team of Support Coordinators assisting NDIS participants and their families to interpret and navigate NDIS plans, establish supports, and increase capacity to exercise choice and control. Key Responsibilities Understand and abide by the NDIS Code of Conduct to promote and protect participant safety Deliver support in a manner that meets the quality standards set out in the NDIS Practice Standards and Quality Indicators Help participants connect to NDIS and other supports Build a participant’s capacity and capability to understand their plan, navigate the NDIS and make their own decisions Broker participant supports and services Monitor participant plan budgets and support effectiveness Crisis planning, prevention, mitigation and action Maintain and develop relationships with key stakeholders and networks About You Lives the Stellar Support Coordination values of Respect, Empathy, Integrity, Equality, and Empowerment Displays strong empathy and interpersonal skills Has excellent verbal, non-verbal, and written communication skills Is a self-starter and works independently with confidence Fosters collaborative relationships with colleagues in-person and remotely Always acts ethically and in the best interest of participants and their families Role Requirements Previous experience working as a NDIS Support Coordinator (12 month minimum) Current driver's license and willingness to travel to clients where required Willingness to undergo NDIS Worker Screening Check Current Working with Children Check Stellar Support Coordination is an NDIS registered business based in Albury Wodonga serving the local area & surrounds, as well as the Greater Bendigo region. We started the business after hearing many stories about people with disabilities not being able to access the NDIS, whilst those who could access the system found it difficult to understand and navigate. Our aim is to encourage our participants to be creative and innovative with their funded supports so that we can coordinate the services they need to achieve their goals and live the life they deserve.
