Principally on-site role for a Community Support Worker based in Campbelltown, SA. The Community Support Worker will assist individuals by providing community-based support services, developing and implementing case management plans, and ensuring that clients have access to resources and services that meet their needs. Responsibilities include building relationships with community members, working collaboratively with service providers, and advocating for clients' well‐being and inclusion within the community. The role requires proactive engagement and the ability to work effectively with individuals from diverse backgrounds. Responsibilities Assist individuals by providing community-based support services. Develop and implement case management plans. Ensure clients have access to resources and services that meet their needs. Build relationships with community members. Work collaboratively with service providers. Advocate for clients' well‐being and inclusion within the community. Qualifications Strong interpersonal skills and the ability to build trusting relationships. Proficiency in community support and community health services. Excellent communication skills, both verbal and written. Experience with case management, including assessment and planning. Ability to work collaboratively with community organizations and stakeholders. Organizational and problem‑solving skills to manage multiple priorities. A passion for supporting underrepresented and vulnerable individuals. Certificate III or IV in Community Services, Social Work, or a related field, or equivalent qualifications would be great but not essential.
