Care Coordinator – Paediatric Community Led Integrated Health Clinic Located in Churchill, full‑time, fixed term until 30 June 2026. Salary: $48,176.3 – $55.98 per hour. About The Role As a Care Coordinator, you’ll play a vital role in supporting children and families to access the care and services they need. Working closely with healthcare and social service professionals, you’ll coordinate care, build family capacity, and help ensure every child receives the best possible start in life. Day‑to‑Day Responsibilities Coordinating intake, assessment, care planning, and referrals. Providing family‑centred support and helping parents navigate health and welfare systems. Monitoring progress and adjusting care plans to achieve optimal outcomes. Collaborating with the Berry Street Social Services Navigator and other key partners. Driving continuous quality improvement in a multidisciplinary clinic environment. This is an opportunity to use your clinical and coordination skills in a rewarding, community‑based setting – supporting some of the most vulnerable families in the Latrobe Valley. About You You’ll Bring A degree in Allied Health or Nursing with current AHPRA registration. Experience in paediatric healthcare, with knowledge of child development and family‑centred practice. Strong communication skills and a passion for working with children and families. The ability to work collaboratively and independently within an integrated care model. A Victorian driver’s licence and willingness to undergo relevant pre‑employment checks. Desirable: Postgraduate qualifications relevant to paediatrics and/or experience implementing new programs.
