We partner with GPs to provide Home Medicines Reviews (HMRs), ensuring patients receive personalised medication management in the comfort of their homes. Position Overview We're looking for HMR-accredited pharmacists to join our expanding team. In this role, you'll make a real impact on patient care by conducting comprehensive medication reviews and working closely with GPs. Note : Chemist2U referrals are currently active across QLD, NSW, WA, SA, and TAS, with limited availability in VIC at this stage. Key Responsibilities Book home visit appointments with patients referred for HMRs. Conduct thorough HMRs, assessing the safety, effectiveness, and appropriateness of prescribed medications. Prepare detailed HMR reports for the referring GP, highlighting findings and recommendations. Maintain strong communication with the Chemist2U support team to ensure seamless service delivery. Qualifications & Requirements Accredited with the Australian Association of Consultant Pharmacy (AACP) and HMR accredited. Current registration with the Australian Health Practitioner Regulation Agency (AHPRA). Professional liability and public indemnity insurance. Previous experience conducting HMRs in a community pharmacy setting. Strong written and verbal communication skills. Ability to work independently while fostering collaborative relationships.
