As a CNS2 level Wellbeing Nurse provides clinical leadership, has extensive health system knowledge, and operates with a high degree of autonomy and independence in an interagency context. This role works in identified local primary and high schools, applying knowledge to navigate health and social systems and provide integrated health care coordination. Responsibilities include: Promoting the Wellbeing Nurse to school partners, engaging with stakeholders, and establishing robust consultation and referral networks. Providing high‑level care coordination, ensuring access to broad health and psychosocial supports across multiple agencies. Leading health care coordination of referred school students and families by facilitating identification of needs and connecting clients to appropriate services for timely interventions. Activities noted in the NSW Wellbeing Health In‑reach Nurse Guidelines, guiding local implementation and interagency care coordination.
