Together, let's make a difference The opportunity Are you a skilled Registered Nurse ready to take the next step in your career? Silverchain is seeking a passionate Case Coordinator to lead a clinical team in delivering integrated community care to clients with complex acute and aged care needs. In this pivotal role, you will coordinate clinical and physical resources, provide direct client care, and serve as a key escalation point for clinical challenges. You will also collaborate closely with the Clinical Nurse Manager to achieve service delivery goals and uphold Silverchain's commitment to excellence. This role operates across Monday to Friday, between 7am and 4pm. Key responsibilities Lead and support a dedicated clinical team, fostering growth, competence, and emotional wellbeing Coordinate resources to deliver person-centred, evidence-based care that meets client needs Deliver hands‑on clinical care and resolve complex issues with confidence and compassion Act as a key escalation point for nurses managing client deterioration and other clinical challenges Drive communication through team meetings, stakeholder engagement, and documentation excellence Support onboarding, mentoring, and student placements to build a strong, future‑ready workforce About you You have passion with purpose and are driven by excellence.
