Social Worker Level 1/2 - Shoalhaven Transitional Aged Care Service Be pivotal to the coordination of patient care to ensure optimal health outcomes for elderly clients in the community in collaboration with a multidisciplinary team. Salary : $38.51 - $54.94 per hour + 12% super & salary packaging | Permanent Part Time 16 hours per week | Shoalhaven Memorial Hospital (SMH) | Applications close: 11 December 2025 Provide high quality social work service to patients, clients, consumers and their families/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct. Key Responsibilities Provide the above social work service to the Illawarra Transitional Aged Care Service (ITACS). Maintain social work and case management responsibility for a number of cases until discharge. Early psycho-social assessments, care planning, education and advocacy for the needs of clients, families and/or carers. Liaise with community agencies, the client's General Practitioner, counselling and carer support. Work as part of an inter-disciplinary team providing low intensity therapeutic home‑based programs to frail elderly clients ready for discharge from sub‑acute hospital care. Qualifications & Requirements Applicable vaccination requirements for a Category A role. Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
