ITACS provides a time‑limited, goal‑oriented service for older people who would benefit from additional low‑intensity therapy in their own homes as well as assisting with the decision‑making process around long‑term care service supports. As the Social Worker in ITACS you will provide social work and case management responsibility for a number of cases from admission to discharge. You will provide early psycho‑social assessments, care planning, education advocacy for the needs of clients, families and/or carer's, liaise with community agencies, the clients General Practitioner, counselling and carer support. The position functions as part of an inter‑disciplinary team with a teamwork approach to the provision of low‑intensity therapy home‑based programmes to frail elderly clients who are ready for discharge from sub‑acute hospital care but are unable to return home without additional care and support. As a part of the new Shellharbour Hospital and Integrated Services Project, a new Community Health Centre is being built on the old Port Kembla Hospital site. Located on Dharawal Country, Warrawong Community Health Centre will bring together a range of services including community mental health, drug and alcohol, community nursing, and child and family services.
