Where you'll be working The position is within the Social Work department and is part of a wider, dedicated multidisciplinary team providing acute and sub‑acute care to patients at Ryde Hospital. Ryde Hospital is having an incredible $479 million complete hospital redevelopment that will transform the delivery of healthcare for the Ryde local community. The Ryde Hospital Redevelopment will result in new health facilities and enhanced services including emergency, ICU/critical care, inpatient units, outpatient/ambulatory care services and much more. Construction commenced in early 2023, and will provide you with a wonderful and exciting opportunity to be part of the Ryde team to change the future health services for the Ryde Community completely. What you’ll be doing This full‑time, permanent position provides an opportunity for an enthusiastic clinician to share their skill and experience in working with older adults in an acute hospital environment. The successful applicant will provide Social Work interventions including psychosocial assessments, counselling, advocacy, crisis interventions and complex discharge planning to inpatients of Ryde Hospital. They will lead quality improvement initiatives and provide education and mentoring to the multidisciplinary team around age care issues. The Ryde Hospital Aged Care Senior Social Worker, while having a speciality in all clinical aspects relating to aged care, may be required to provide service to any area within the hospital, including participation in an after‑hours on‑call roster. They may be required to work weekends, after hours and public holidays. People of Aboriginal and/or Torres Strait Islander background are encouraged to apply. Candidates will need to meet the following criteria: A Bachelor or post‑graduate degree in Social Work and eligibility for the Australian Association of Social Workers (AASW). Evidence of extensive clinical experience, inclusive of experience as a Level 2 Social Work. Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation process. Experience in clinical teaching and supervision of students, level 1/2 Allied Health Professionals and/or Allied Health Assistants. Demonstrated advanced skills and knowledge in the provision of current social work practice, particularly in regard to aged care issues. A strong commitment to an evidence‑based service to provide for consumers, carers and families requiring assessment, case management, intervention, appropriate referral and discharge planning. Demonstrated understanding and experience working with the impact and management of trauma, sudden death, bereavement, domestic violence, elder abuse, child protection and Guardianship. Demonstrated proficiency in ability to work autonomously with limited supervision. Have the ability to prioritise work, manage a demanding workload, apply high level reasoning skills and have independent professional judgement.
