| | || --- | --- || Workplace Location | Brisbane - West,Ipswich region |**About the role**The Clinical Nurse Consultant (CNC) is an expert clinical practice registered nurse who is responsible for planning coordinating and providing advanced breast care knowledge, education and support to patients with breast disease from the point of diagnosis, through treatment, rehabilitation, follow up, palliative care in accordance with recognised clinical practice guidelines. The position will work within the multidisciplinary model of cancer care and work in partnership with service providers in acute and community settings, promoting breast awareness.This role provides a service which incorporates the rural communities in Gatton, Esk and Boonah through the provision of education and information about breast awareness clinics and assisting breast cancer support groups.**About you**Our ideal candidate will be someone who can demonstrate the following:* Demonstrated high level of competence and expertise in advanced practice with respect to the management of women with breast cancer/oncology nursing/women's health within acute and community settings.* Demonstrated ability to provide advanced clinical skills in counselling and health education to the multidisciplinary team to the client, their families and the community.* High level verbal and written communication skills together with high level interpersonal and organisational skills.* Demonstrated ability to initiate, plan and evaluate local service improvement with commitment to total quality framework, management and improve health outcomes through project management, organisational and planning skills.* Exemplifies the West Moreton Health Professional Practice Model by demonstrating person-centred care, professional excellence, safe and effective care delivery, commitment to quality and safety, and collaborative practice.**Mandatory Requirements:*** Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the ( )* Appointment to this position requires proof of qualification and registration or membership with the Australian Health Practitioner Regulation Agency (Ahpra).
