After Care Coordinator

Holyoake is a leading provider of counselling and support services for people impacted directly and indirectly by alcohol, other drugs, and related mental health issues. We operate across multiple locations in Western Australia, and employ over 100 staff. Holyoake’s Wheatbelt Community Alcohol and Drug Service is seeking a passionate and empathetic health professional to support individuals experiencing suicidal ideation or suicide behaviour throughout the Wheatbelt in the position of After Care Coordinator in Suicide Prevention (ACC). This position will be primarily based in Northam on a 0.8 FTE roster (30 hours per week) and involves outreach throughout the Wheatbelt region. Consideration will be given to suitable applicants living in or within close proximity to the Wheatbelt. The After Care Coordinator will work closely with Holyoake’s Northam Medicare Mental Health Team, Wheatbelt Suicide Prevention Coordinator, and Wheatbelt Community Alcohol and Drug Service team as well as other key stakeholders within the Wheatbelt. Key responsibilities Undertake direct liaison between the consumers of the After Care program and the discharge staff at hospital emergency departments to ensure continuity of care. Upon referral and discharge from hospital, provide case management for the consumers to co-ordinate their care as detailed by the hospital discharge plan. Identify any other supports required including social and practical support, reconnection to their family and/or support networks, GP and other primary care services. Provide support to the client in a range of locations that may include on-site and community locations, observing the requirements of Holyoake’s Outreach Policy. Provide support and encouragement as required, to facilitate the achievement of the discharge plan. Provide leadership and guidance to the After Care Support Worker. Manage allocated client caseload in accordance with Holyoake clinical guidelines. Maintain membership of the Wheatbelt Suicide Prevention Advisory Forum to provide updates on service provision, trends, areas of concerns, gaps in service provision/cohesion. Contribute to the review and quality improvement of Holyoake services. Provide feedback on service gaps to the Services Manager and the Wheatbelt Suicide Prevention Coordinator. Promote the service and provide information and presentations to key stakeholders as required. Skills and experience required A tertiary qualification in social work, psychology, mental health (nursing), counselling or equivalent experience. Demonstrated experience in working to support individuals with mental health concerns and suicidal ideation. Demonstrated experience in conducting suicide risk assessments, de-escalation, and the development and implementation of client safety plans. Demonstrated experience in complex case management. Sound knowledge of alcohol and drug issues, mental health conditions and their comorbidity. Minimum 4 years professional experience in a related field.
