You will be working as a team member with the Transitional Aged Care Prgram (TACP). This a home based rehabilitation program for older people who have been admitted to hospital and ready for discharge home.
Transitional Aged Care Program (TACP) comprises of Allied Health and Nursing team members and three Community Aides. It is based with the Prince of Wales Community Health department.
Where you'll be working
You will be based with the Prince of Wales (POW) Community Health. This position is a part time Dietitian with the POW Transitional Aged Care Program (TACP). You will use hospital Fleet vehicles to visit clients in their home. You are the only Dietitian in this team. However, you be part of the general Community Health team.

Plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within the Dietetics discipline in order to provide high quality Dietetics care to clients with the Transitional Aged Care Program, consistent with NSW Health, South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.
This role includes responsibilities for supervision, contributions to quality improvement initiatives, education delivery to other health care professionals and the teaching of students on clinical placement.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified for this position following suitable labour market testing.
All Category A workers and new recruits are required to receive one dose of the seasonal influenza vaccine annually to be considered compliant.
Category A workers and new recruits who are non-compliant with seasonal influenza vaccination or have a medical contraindication to influenza vaccinations must comply with all other infection control risk reduction strategies as directed while working in a Category A position.

Selection Criteria:
- Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award) that provide eligibility for full membership of Dietitians Australia and demonstrated commitment to ongoing professional learning.
- Extensive post graduate clinical experience in Community and/or Aged Care services.
- Excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
- Demonstrated ability to provide high level clinical advice, education and support to others in providing service excellence.
- Ability to work as part of a multi-disciplinary team and to apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel, complex or critical nature.
- Demonstrated ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes.
- Demonstrated active participation in developing, implementing, and reporting on services.
- Current drivers licence (or a willingness to travel in accordance with the demands of the position).
