Aboriginal Connection Program Case Manager Join Our Mission: Aboriginal Connection Program Case Manager - SA Health Join SA Health as an Aboriginal Connection Program Case Manager, providing culturally appropriate case management to Aboriginal clients accessing drug and alcohol services in Adelaide CBD. This full‑time contract position until July 2026 offers the opportunity to combine clinical expertise with cultural understanding to deliver person‑centred care that respects traditional healing alongside contemporary healthcare. About the role: Reporting to the APY Manager, this role is responsible forthe delivery and evaluation of a culturally sensitive and comprehensive evidence‑based specialist assertive outreach alcohol and other drug service to individuals with drug related problems, their families and significant others. The role involves undertaking assessment, counselling, evaluation and client follow‑up, provision of information advice and assistance to the general community. Duties for the role will include: Assisting in the provision of home visits to clients, including monitoring client's wellbeing, client and family consultation, assessment, support, counselling, referral, and follow‑up on a range of health issues and clinical screening services, Maintaining clients' confidentiality, accurate, up to date and legible client records and statistical information concerning activities from which to develop and produce relevant data, client service statistics and reports, Monitoring client intervention plans to ensure appropriate client care outcomes are achieved, and that client rights are upheld, Using culturally accountable practices that respect Aboriginal lifestyles, values, identity, and culture, Planning, organising, and monitoring interventions implemented to meet the client's needs in both the long and the short term, Facilitating case meetings with client on a frequent regular basis involving other relevant agencies and family as indicated, Formulating, recording, and managing individual case plans, Taking responsibility for the care, plan and goals set in collaboration with the client, Providing support, information and advocacy to community groups to address alcohol and other drug issues, Foster community participation in the planning, implementation and evaluation of health promotion programs, Promoting the community role, provide DASSA representation, and disseminate resources in the local service area About you: Ability to communicate effectively with Aboriginal people, Ability to provide effective and sensitive advice and assistance to people in crisis and conflict situations or other hardship, Ability to facilitate planned care for Aboriginal clients in a community setting, Demonstrated experience with Case work including effective assessment, intervention, case management and referral skills, Proven experience in Microsoft Applications and basic computing skills, including email, database entry and word processing, Hold, or working towards gaining, or is prepared to undertake training to attain the Certificate IV Alcohol and other drug studies or an equivalent qualification, Post graduate qualification relevant to community settings, DASSA client group or communities Please see the attached Role Description for additional information.
