Today, the Foundation continues his legacy, delivering frontline services and community support across NSW, ACT & QLD. With over 200 staff and 350 volunteers, we remain deeply committed to helping young people affected by issues related to drugs, crime, and mental health. The Role We are looking for a dedicated and compassionate Case Manager to join us in providing support for young people who are participating in this new early intervention program. The program is intended to help youth demonstrating high-risk behaviours, such as substance abuse, aggression or truancy, and their family by providing a short stay (Reset), as well as mentoring and therapeutic supports for a period pre and post stay. Responsibilities Delivering case management services for young people in the program. Conducting assessments and developing Case Management Plans. Providing support for clients to assist them in meeting their goals. Actively maintaining contact with young people in the program such as case work, mentoring, and group work. Providing direct support during the Resets including potential after-hours or weekend work. Working closely with the team for day-to-day implementation of care plans. Ensuring young people have proper representation and access to all the services they require. Arranging community networking for the young person to encourage positive interactions in a safe and supportive environment. Liaising effectively with referrers, service providers, key stakeholders, and/or family and carers. Maintaining accurate and up to date client records, statistics, and reports. Working closely with Adolescent and Family Counsellors and Youth Workers to coordinate the provision of care for young people. Developing and maintaining a varied and viable program of activities for young people including individual tailored programs and group activities. Demonstrate a commitment to culturally safe practice, particularly when working with Aboriginal and Torres Strait Islander young people and families. Qualifications Experience and demonstrated ability to work with young people between the ages of 12 to 25 years old. Minimum 2 years direct casework experience. Experience in the drug and alcohol sector. Lived or employment experience in a therapeutic community. Excellent written and oral communication skills. Ability to work effectively both as a team member and as a practitioner with minimal supervision. Strong personal and interpersonal boundaries. Commitment to the principles of harm minimisation and to the aims of the Ted Noffs Foundation. Must hold and maintain a cleared QLD Blue Card and a current DriverLicense (provisional license minimum).
