We provide care to those in aged care, post-operative care and people living with disabilities. Our services help clients remain safe and independent in their own homes. About the role We are seeking to recruit dedicated individuals who strive for best practice, are able to communicate well with people of all backgrounds and approach every situation with care and compassion. As our Care Coordinator, you will be responsible for : Building strong connections with clients and their families Assessing client needs and resources for health, welfare and other facilities and services Conducting Client Care Assessments and Care Planning Maintaining accurate records as per the Aged Care Quality Standards and NDIS Practice Standards, Quality and Safeguarding Framework Supporting all clients to meet their goals Supporting and providing feedback to Home Care Workers Re‑organising work priorities to meet the needs of a fast‑paced environment Must promote the values of the company and prepared to plan supports using a contemporary approach Essential Criteria Experienced in assessing and planning care needs 3 years Support Worker experience Sound understanding of the Aged Care Quality Standards and NDI Practice Standards, Quality and Safeguarding Framework Organisational, administrative and time management skillsAll candidates must have the right to work in Australia COVID 19 Vaccinations Ability to Obtain National Police Check Blue Card Drivers Licence NDIS Worker Screening Check First Aid & CPR Certificate Check us out at Job Type: Full‑time Salary: From $75,000.00 per year Schedule Monday to Friday On call Education Certificate I - IV (Preferred) Licence / Certification National Police Check (Preferred) COVID Vaccination (Preferred) Work Authorisation Australia (Preferred) Work Location: In person Qualifications National police check Current First Aid and CPR Reliable source of transportation required Comprehensive Car Insurance If EN - Current AHPRA registration & Professional Indemnity Insurance #J-18808-Ljbffr
