About The Role Based within the newly established Lower Mid‑North Coast (LMNC) Hub, this role offers a unique opportunity to provide specialist coordination and leadership in palliative and end‑of‑life care. You’ll work collaboratively with a dedicated multidisciplinary team — including nursing, allied health, medical specialists, volunteers, and GPs — to ensure patients and families are supported through every stage of their journey, in the environment that feels right for them.
