Nurse Practitioner – Residential Aged Care | Central Coast, NSW A rare opportunity for an experienced Nurse Practitioner to provide high-quality care to residents within Aged Care Facilities on the Central Coast. This role offers 1 day per week, ideal for someone seeking flexible work or a portfolio position alongside other commitments. About the Role You’ll collaborate with the Senior Medical Officer and GPs to deliver best-practice clinical care and help reduce hospital admissions through proactive management. You’ll manage your own caseload of RACF residents, coordinating their ongoing health reviews and medication plans. Key Responsibilities Weekly on-site visits to assigned aged care facilities Conduct clinical assessments, reviews, and preventive health care Coordinate with GPs, staff, and families for continuity of care Contribute to medication management and annual health assessments About You AHPRA-registered and endorsed Nurse Practitioner Eligible for Medicare Provider Number Collaborative, flexible, and confident working independently Excellent communication and patient rapport What’s on Offer 1 day per week – flexible scheduling Supportive team with clinical leadership from GPs and SMO Stable ongoing work within a well-structured organisation Looking for a flexible side opportunity that fits around your current commitments?
