Aged Care Service Navigor Care Finder Program

Catalyst Foundation is a not-for-profit organisation and the leading provider of community information in South Australia. Our mission is to help the aged and vulnerable community by connecting people to services and providing relevant, independent information to all South Australians.
The Royal Commission into Aged Care Quality and Safety highlighted the complexities of the aged care system, noting that some individuals find it more challenging to navigate and access necessary services. In response, the care finder program was established as part of the Connecting Senior Australians to Aged Care Services measure announced in the 2021-22 Commonwealth Budget.
As a Care Finder, you will provide specialist and intensive assistance to help individuals within the care finder target population understand and access aged care, as well as connect with other relevant supports in the community.
Some of the functions of a Care Finder include:
- Supporting individuals to interact with My Aged Care for eligibility screening and assessment referrals.
- Guiding and explaining the assessment process, including attending assessments when appropriate.
- Helping individuals find the aged care supports and services they need and connect with other relevant community supports.
We would love to meet you if you have:
- Relevant qualifications (e.g., social work, human services, aged care, community services, or health) and/or relevant experience.
- Local community connections and the ability to leverage Catalyst Foundation's connections with the care finder target population.
- A detailed understanding of the range of aged care supports and services available in the local community.
- An understanding of the process to access aged care supports and services, including My Aged Care eligibility requirements.
- A commitment to delivering a person-centred approach that respects individual needs, preferences, values, and life experiences.
- The ability to communicate effectively with a broad range of people, demonstrating active listening and empathy.
This role is a 0.6 FTE position and is under the Social, Community, Home Care and Disability Services Industry Award.
