Clinical Case Manager (Registered Nurse) | Aged Care | Canberra
Permanent full-time opportunity for a Clinical Case Manager to work directly for one of our clients in Canberra
Paying up to 100kOn-going career progressionSalary packaging with novated leasing options and retail discountsMonday - FridayAbout the role Assessment and Care PlanningOversee the delivery of care to residents, ensuring their physical, emotional, and social needs are consistently met.Develop and implement personalised care plans in partnership with residents, their families, and healthcare professionals.Maintain accurate records of care plans, government-funded home care packages, health status changes, and any interventions provided.Resident EngagementCollaborate with key organisational stakeholders to address and support resident care needs.Assess eligibility and manage residents' access to government-funded care programs.Work with the lifestyle team to design and implement social, community service, and activity programs that align with residents' goals and enhance community connection.Stakeholder EngagementPartner with doctors and allied health providers to ensure the continuity and quality of care for residents.Required Bachelor of NursingCurrent AHPRA registrationDemonstrated experience as a Registered Nurse within aged careKnowledge of relevant legislation, regulatory requirements, professional standards, policies and proceduresStrong written and verbal communication skillsNational Police and NDIS clearancesFlu vaccination and Covid vaccinationsWhy apply through Healthcare Australia?
