Clinical Case Manager - Home Care

Help clients maintain their independence in their own homes while leading a team of experienced care professionals.
Benefits: Competitive Salary: Enjoy a fantastic package with additional salary packaging benefits.Work-Life Balance: Flexible Monday to Friday working hours with a hybrid working location.Autonomy: Plenty of independence in your role to make a genuine impact.Mentorship: Opportunity to work with an experienced manager who offers support and guidance.Career Growth: This progressive role comes with excellent future growth opportunities.Your Responsibilities: Build and maintain strong relationships with clients and their families.Ensure quality and compliance by conducting regular visits to clients' homes.Assess client needs, create & develop individualized care plans, and evaluate their effectiveness.Collaborate with the Rostering team to ensure the right skill mix is provided for clients.Develop and sustain connections with internal and external stakeholders to deliver a range of services.What We're Looking For: Experience: Minimum of 1-2 years in the Home Care industry.Qualification: Cert IV in Aged Care or equivalent qualification.Skills: Proven ability to build strong community networks and relationships.Knowledge: In-depth understanding of the Aged Care sector, including Home Care Package guidelines and service requirements.If you're passionate about making a difference and advancing your career in a supportive environment, we'd love to hear from you
