Case Manager – ACT and region Due to significant growth, we are looking for a Case Manager
This is an exciting opportunity to work with an independent, not-for-profit organisation located in Mawson, working with a small, friendly, and client-focused team.
You will be involved in Case Management with our Home Care Package clients, service promotion, providing advice on access to community-based aged care, and working closely with our direct Support team members.
We also provide Short Term Restorative Care, CHSP Other Food Services, brokerage to other agencies, and private fee-for-service.

About you: Demonstrated previous experience in client assessment and Case Management in a community setting, preferably in aged care. Sound understanding of all relevant legislation as well as the Single Funder Model being introduced in mid-2025. Demonstrated ability to work collaboratively within a small team environment. Demonstrated knowledge of, and ability to access, a range of community resources and professional services for clients. Demonstrated ability to work effectively with a culturally diverse group. Demonstrated ability to work through challenges in systematic and creative ways. Demonstrated ability to work autonomously with a high degree of initiative and attention to detail. Demonstrated ability and confidence to actively promote services to build the client base is highly desirable. Excellent communication skills, both written and verbal. Proficient in the use of computer software: Word, Excel, Internet, email, and client databases. Demonstrated commitment to the 'Purpose and Principles' of Annecto – the people network. A current valid Australian driver's license. What would be great to have? Experience using Client Management Systems such as Techone, Alayacare. An understanding of the ageing process and its cultural impact. An ability to speak a community language/s.
