Calvary Home Care - Care Manager Make a difference at one of Australia's leading health care providers and within our local communityGenerous salary packaging benefits, and discountsOngoing professional development and advanced career opportunitiesPermanent Part Time - 4 days a weekAbout the role The Care Manager is responsible for delivering case management that is person-centred and enablement-focused, in accordance with Calvary Home Care policies and procedures, program guidelines, and the National standards of practice for Care Management and the National Code of Ethics and Calvary Home Care Management Model.
Key Responsibilities Time management and working within expected time framesUnderstanding the Aged Care StandardsLiaising with stakeholders and internal networksMonitor and respond to changes in clients' requirements with a person-centred approachUtilization of innovative and problem-solving skillsWhat you bring Demonstrated experience in the provision of Case Management servicesDemonstrated experience in the aged/community/disability sector particularly working with clients with complex needsDriver's licenseHighly developed proficiency using computers including the Microsoft Office suite with the ability to learn and adapt to new databasesUnderstanding of the financial principles of individualised client funding and experience in monitoring and reviewing individualised budgetsAn ability to work in a team and negotiate with internal and external stakeholders to ensure accountability for service deliveryNDIS clearance (or willingness to obtain one)Police clearance (within the last 6 months or willingness to obtain one)For further information please contact Recruitment on Why work for Calvary?
