We are currently seeking dedicated and compassionate Support Workers to join our team and make a positive impact on the lives of our clients.
Job Description: As a Support Worker at Hearts to Homes, you will provide essential care and support to individuals with disabilities, helping them achieve their goals and maintain their independence.
You will work closely with clients in their homes and communities, delivering personalized support and fostering an inclusive, respectful environment.
Key Responsibilities: Assist clients with daily living tasks, including personal care, meal preparation, and household chores. Support clients in accessing community activities, appointments, and social engagements. Provide transportation and accompany clients to various destinations as needed. Administer medication and manage health-related tasks as per clients' care plans. Encourage and facilitate clients' participation in recreational, cultural, and educational activities. Foster a safe, respectful, and supportive environment that promotes clients' well-being and independence. Communicate effectively with clients, their families, and other members of the support team. Document and report on clients' progress, challenges, and any incidents in accordance with organizational policies. Qualifications and Skills: Certificate III in Individual Support (Disability) or equivalent qualification. Current First Aid and CPR certification. Valid driver's license and reliable transportation. NDIS Worker Screening Check and Working with Children Check (if applicable). Previous experience in disability support or a similar role is highly desirable. Strong communication and interpersonal skills. Compassionate, patient, and respectful attitude towards individuals with disabilities. Ability to work independently and as part of a team.
