About the role
Permanent full time opportunity to provide social work clinical leadership within STARS, requiring the independent application of clinical expertise in a highly complex and varied patient population.
The role is central to ensuring quality discharges and maintaining patient flow within the geriatric and rehabilitation service lines within STARS, which includes patient cohorts with a prolonged length of stay due to various complicating factors, primarily NDIS, transition to RACF, and convalescing patients; inclusive of supporting colleagues and providing clinical advice within scope.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Integrate advanced knowledge, evidence-based skills, and reflective practice in the delivery of clinical services to enable the NDIS pathway, independently applying professional judgement to complex problems.
Deliver advanced clinical services across multiple specialties with minimal supervision.
Apply valid, reliable evidence and high-level professional judgement to complex caseloads and maximize opportunities to reduce barriers to discharge.
Lead quality improvement initiatives for Social Work within STARS and contribute to broader quality initiatives within STARS, MNHHS, and/or state-wide initiatives.
