We are looking for a highly motivated, self-directed and experienced clinician who can successfully work with a team of support staff to provide the best care possible to private, HCP, NDIS and DVA clients in and around Melbourne.
If you have good physical assessment skills, are able to problem solve, and back this up with excellent interpersonal skills and communication, then this is potentially the dream job for you.
ABOUT THE ROLEWe have an exciting opportunity for a motivated, self-directed and experienced clinician to join our team as Care Manager in the South East region, reporting directly to a Regional Care Manager
A Registered Nurse by profession, the Care Manager is a key role responsible for the provision of care and services to a select group of valued clients.This rewarding and hands-on role requires a proactive approach to the planning, coordination, delivery, and case management of our clients' care and services.Working closely with the Rostering Team, the role includes engaging with and leading our care team, consisting of Nurses and Carers.Full-time position, working with Care RN's and an afterhours team that covers weekend on-call with weeknight call shared on a rotational basis amongst the CMs.This role is based at our office in Hawthorn but will be working with our clients.ROLE REQUIREMENTSUnrestricted registration with AHPRA as a Registered Nurse.A minimum of 3 years recent clinical, care or case management experience in an aged care or community setting.A current full driver's license and a well-maintained, registered vehicle.The right to work in Australia & ability to obtain a satisfactory Police Check.WE'D LOVE TO HEAR FROM YOU IF YOU MEET THE ABOVE ROLE REQUIREMENTS AND:Are passionate about the highest standard of care and service delivery.Have excellent communication and organisational skills.Are self-directed, an excellent problem solver, and a team player.Are personable, customer-focused, and well presented.Are proficient in Microsoft Office and other software programs.Candidates will be highly regarded if they have a formal qualification in Care or Case Management and/or sales and service experience in a related field.WORKING WITH USAs an employee of Luxe Care, not only will you be part of an organisation that makes a real impact on people's lives, but you will also be offered a range of benefits such as:
