The successful post holder will function as a Mental Health Clinical Nurse as part of a multidisciplinary team responsible for coordinating and delivering case management and care coordination activities. The Clinical Nurse will manage a case load that supports individuals with serious mental illness to provide specialist assessment, crisis management, care coordination, delivery of nursing interventions, case review, advanced care planning and risk management to the Continuing Care Team.
This role will also contribute to the planning, development, implementation, maintenance and evaluation of a broad range of treatment and prevention activities to promote recovery from mental illness.
About the Team The Continuing Care Team (CCT) works individually and in group settings to provide ongoing mental health interventions.

To promote and maintain mental and physical wellbeing for all consumers.
To ensure that services are accessible and responsive to individual needs.
To ensure that any intervention assists the individual maintain an engaging and self-determined lifestyle in the community.
To advocate for the consumer who may be experiencing stigma or marginalisation as a result of mental illness and psychiatric disability.
To educate and raise awareness of serious mental illness to the wider community.
